History Form

The history form was designed for new patients to speed up their wait time at the office. The form was designed in html, and can be printed of your computer. For optimal printing please set your screen area to 1024 by 768 and in internet explorer please set your text size to "smaller". For step by step instructions on how to do this please view the "Get some Help" page.

Instructions: Click the link at the bottom of the page to open the history form in a new window. Print the history form by selecting "file" then "print". Wait a few minutes for the ink to dry, then fill out the history form and bring it with you to your visit.
